Introduction- Idiopathic polypoidal choroidal vasculopathy is a disease of the middle aged and above.It is considered a phenotype of ARMD. Materials and method-A young lady of 35 years presented with OD sudden drop of vision. She had subretinal hemorrhage and hemorrhagic PEDs obscuring macula.FFA, ICG did not show CNVM or polyps.Depending on the presentation a presumptive diagnosis of IPCV was made. She was managed medically with Anti VEGF s alone with good clinical outcome. PDT was not required as the response was satisfactory. Conclusion -IPCV can present at early age also. Polyps may not be always visible. The response is good to Anti VEGF s. However it may be supplemented with PDT.
