Cases to report for surgical management of advanced necrotizing viral keratitis, delayed medical management and loss to follow up. Case One: A 32 year’s old man diagnosed as a case of bilateral recurrent viral keratitis with perforation of right cornea. A therapeutic keratoplasty was performed. He discontinued regular follow up. Later revealed total corneal opacity , phthisical right eye with no residual symptoms. Case Two: A 25 years old lady was diagnosed as total sloughed corneal ulcer with iris prolapse. . Theraputic penetrating keratoplasty done with large graft. Lately graft perforation occurred due to persistent epithelial defect.The first case with improper medication and irregular follow up resulted in a quiescent symptom free but the Second case that received maximal medication and surgical therapy unfortunately could not be saved. In both cases relentless progression of viral keratitis is demonstrated, emphasizing for early diagnosis and treatment.
