INTRODUCTION: A Giant MC is a rare nodular variant of MC with diameter
of more than 0.5-1cm. It is rare & seen mostly in immunocompromised patients.
CASE SUMMARY: A 45 year old Retropositive male presented with a large left upper eyelid abscess like swelling. But on examination a single, firm, lobulated, non tender mass of 1.5 cm x 1.4 cm was noted apart from multiple umbilicated papular lesions on face and both eyelids. With a meticulous excision mass was removed in toto along with removal of few more lesions. Grossly it appeared to have multiple lobes and gyri. Histopathology showed hyperkeratosis with multiple intracytoplasmic inclusion bodies consistent with MC. The patients CD4 count was 124 cells/ul.
DISCUSSION: In immunosuppressed patients MC has a chronic course & rarely can become large as a giant MC. CD4 lymphocytopenia is postulated in its pathogenesis. Giant MC must be kept in mind while evaluating an immunosuppressed patient coming with large abscess like lid lesions.
