Anterior chamber becomes deep and angle of AC opens out following phaco for various reasons like thicker senile cataracous lens being replaced by thinner IOL, distension of AC by pressure of irrigating solution and visco etc. In SICS there is hardly any effect of irrigating solution or visco. So it is presumed that deepening of AC and widening of angles may be less in SICS group. But we observed something different. ACD and angle of AC were measured by anterior segment OCT before and after one month in 58 eyes. In 30 eyes phaco was done through 2.2 mm CS incision with Rigid IOL implantation (incision enlarged up to 5.5 mm) while in 28 eyes SICS was done and Rigid IOL was implanted through 5.5 mm incision. ACD was increased more by 0.06 mm (2.42%), Nasal angle increased more by 3.530 (14.95%) and Temporal angle increased more by 1.390 (9.58%) in SICS group as compare to phaco group. While IOP reduced up to 4 mm of Hg in both the groups. This paper will discuss the possible explanations.
