Metastatic orbital tumours are uncommon accounting for 0.5-5% of all orbital tumours.
A 55 yrs male presented with chief complaint of proptosis of right eye and upper neck swelling on right side for last one month which showed upper right cervical lymphadenopathy.There was weight loss of more than of 10kgs. Mild proptosis (Axial) present with vision 6/6 in right eye.CECT of orbit and brain- Mass lesion of floor of anterior cranial extending into the right orbit and sinus .Enlarged level II and V nodes are seen on right side.
FNAC of swelling(lymph node) in right upper cervical shows small cell carcinoma.
CECT Thorax and Bronchoscopy reveals small cell carcinoma of lung.
Diagnosis-Patient was diagnosed with extended small cell lung cancer (Stage IV).This case is rare since early symptom was mild proptosis only and no other significant lung signs. Management-Patient was started chemotherapy and concurrent radiotherapy to orbital and brain metastasis showing improvement.
