Macular telangiectasia type 2 is a degenerative disease of the retina. The pathogenesis of this condition remains poorly understood. Primary Muller cell degeneration leads to subsequent vascular changes that are characteristic of the disease. Foveal detachment is an unusual manifestation of the disease and has been mentioned in isolated case reports. We describe four cases of non-proliferative macular telangiectasia type 2 with bilateral foveal detachments in the background of non-proliferative diabetic retinopathy. There was little change in the morphology of the detachments or the visual acuity over the follow up period. One patient received intravitreal injection of Avastin with no significant effect. We can postulate that in the background of diseases like diabetes and hypertension which can compromise the blood retinal barrier function, the manifestations of MacTel can be as dramatic as causing a foveal detachment
