Introduction- Of late various immunomodulatory agents like cyclosporin and tacrolimus have been tried to treat VKC. Two types of tacrolimus ointment are available with different concentrations for the same. We have tried to compare the two. Materials and methods- Tacrolimus 0.1% and 0.03% were compared in terms of efficacy and side effects. The objective and subjective scoring system for efficacy and adverse effects were used to compare. Results- Both the groups showed effectiveness after some weeks. Efficacy wise 0.1% concentration was better but the side effects were much more. The compliance with higher concentration of tacrolimus was much less comparatively. Patients were lost on follow up or moved back to steroids. Conclusion- Both the forms of tacrolimus show good response in long term. The side effects are much more with 0.1%.Its better to start with 0.03% and move only to 0 .1% if response is not adequate
