A 35 years old female presented with painful axial proptosis of LE with limitation of upgaze for several months. Fundus of LE showed choroidal folds and PED over macula with elevation of inferior fundus due to some mass effect. She was diagnose as a case of left IR myocysticercosis without any neurological lesion via CT scan and B scan. She responded well medically with resolution of symptoms but limitation of motility persisted. Therefore orbital cysticercosis should be considered as differential diagnosis of orbital pseudotumour which can be managed medically if diagnosed appropriately
