Retrospective, interventional case series describing inner retinal surface features on en-face SD-OCT after ILM peeling for MH.44 eyes of 42 patients underwent 23G vitrectomy, ILM peeling and C3F8 gas. Pre and postoperative visual acuity(VA), fundus photographs, en-face and B-scan SD-OCT were reviewed. Follow-up was minimum 6 months. Multiple dark dots along retinal nerve fibers in the area of ILM peeling termed concentric macular dark spots(CMDS) was seen in 18 eyes at 2 months(40.9%) and 24 eyes at 6 months(54.5%) after surgery on en-face scans. This feature was not visible on fundus photographs. B-scan OCT showed dimples in the nerve fiber layer corresponding to each stria. Postoperative VA was not significantly different in eyes with and without CDMS. Mean follow-up was 10.8 months (range 6–24 months).Thus inner retinal defects on en-face SD-OCT frequently occurred after MH surgery when ILM was peeled. This CDMS appearance confirms complete ILM removal but does not preclude good VA
