This is a Case report of a 27 year old known pulmonary Kochs patient on Anti tuberculosis treatment (ATT) who presented with redness, congestion and discharge. It was unresponsive to topical antibiotics. On examination a saucer shaped crater like leison measuring about 7x 3mm in size, 4mm away from the temporal limbus with ill defined margins with greyish area of 2x2mm at the centre of the ulcer suggestive of scleral thinning was seen.The conjunctiva showed dilated and tortuous vessels.There was minimal pain on extra ocular movements. Conjunctival swab smear sent for culture sensitivity had no growth.Scleral scrapings taken from the edge of the ulcer was positive for mycobacterium on Acid Fast Bacilli staining. X ray chest and sputum smear confirmed the diagnosis. He was started on injection streptomycin along with ATT treatment. The scleral abscess showed healing response. We report this Case as isolation of tubercle bacilli from direct scleral scraping,which is very rare.
