PURPOSE : The malignant exophthalmos is a rare condition that doesn’t adequately respond to various forms of treatment, requiring, in most cases, surgical correction.
METHOD : This is a case report of 55 year old lady presented with acute progressive loss of vision in right eye with periorbital swelling with severe proptosis with exposure keratopathy.
She was diagnosed as a case of orbital cellulitis and started on systemic antibiotics and shown no response to treatment.
RESULT : On further investigations TSH was found to be raised on thyroid function test, with bilateral diffuse increase in EOM belly thickness with sparing of tendinous site on USG scanning and CECT suggestive of B/L thyroid associated ophthalmopathy with right malignant exophthalmos with exposure keratopathy. Patient was started on systemic steroids and shown significant decrease in proptosis.
CONCLUSION : A detailed systemic and orbital examination led to the correct diagnosis of thyroid associated ophthalmopathy.
