Aim: to explore the outcome of therapeutic keratoplasty in large recalcitrant corneal ulcers.
Study design: It is a prospective study on therapeutic corneal transplants in 58 eyes of 58 patients, recording average pre-operative visual acuity, average ulcer size, intra operative complications and post-operative clinical outcomes over a period of 3 months.
Results: No significant difference in visual acuity was noted pre and postoperatively (light perception to hand motion). The mean ulcer size was 6.78mm2. Out of 58, 34 (58.6%) cases were positive for fungus, seven (12%) for bacteria, two (3.4%) had mixed culture and 15 (25%) cases showed no organisms. Graft rejection and infection was noted in four (6.8%) and ten (17.2%) patients respectively while graft failure occurred in 12(20.68%) patients.
Conclusion: Therapeutic keratoplasty provides rapid infection control and maintains anatomical integrity of the globe. However it requires a subsequent surgery for visual rehabilitation.
