Purpose:To evaluate the safety and efficacy of a step wise management approach to pseudophakic cystoid macular edema(PCME)
Design:Retrospective Case Series
Methods:16 eyes of patients with PCME after uncomplicated phacoemulsification were included. A step wise treatment protocol was followed. All patients were started on topical nepafenac & prednisolone. After 6 weeks non responders were administered 1cc transeptal triamcinolone acetoniode(TTA) 40mg/ml. Another 6 weeks later patients with refractory PCME were treated with intravitreal injection of 0.7 mg dexamethasone implant(DEX). The injection was repeated if necessary.
Results:13 (81.25%) patients improved with topical therapy alone. 2(12.5%) responded to TTA. 1 (6.25%) needed 2 injections of DEX at an interval of 3 months. Changes in best corrected visual acuity & central macular thickness over a period of 12 months were statistically significant (p<0.05). Conclusion:A step wise management approach may be ideal to deal with PCME.
