Dr.Chandana Chakraborti
A 24- year- old male presented with dimness of vision, redness, photophobia and watering in right eye(RE) for 3 weeks. On the basis of S/L examination,we diagnosed this case as bilateral Lattice Corneal Dystrophy with infective keratitis in RE.Smear and C/S report revealed mixed flora. The patient was treated with topical fortified Vancomycin & Amikacin,5% Natamycin,1% Atropine sulphate and oral fluconazole.Failure in medical management prompted us to do therapeutic PK after 2 weeks. At 6 week, penetrating optical PK,BCVA improved to 20/80.The other eye was managed by adequate lubricant and topical antibiotic prophylaxis in view of minor corneal abrasions following any trivial trauma.
Recurrent erosions in lattice corneal dystrophy may predispose the cornea to microbial infection.Occurrence of mixed floral infection has not been reported in the earlier studies. We report this case so that ophthalmologists remain aware of this rare sight-threatening complication of lattice corneal dystrophy.
