Technology has advanced so much that very high refractive error (especially myopia), which previously could not be corrected by any means, can also be corrected today with the help of phakic IOLs. Phakic IOLs are a good option for correcting any level of refractive error even high myopia of > -20 Diopters. Now phakic IOL are also available which can correct myopia > -20 D. In this study, We want to share our experience of phakic IOLs in a small numbers of patients with high myopia for > -20 D. Over 5 years with a minimum follow ups of 6 months. 12 eyes of 07 patients were implanted with phakic IOLs, who had ACS > 28 mm, after doing thorough preoperative work up. They were followed up post op day 1, 1 week, 1 month & 6 months. They were evaluated for BCVA, IOP, vaulting & complications if any etc. No major complication were observed. One case needed exchange of phakic IOL. Their safety profile was assessed.
