It is a rare form of malignant tumour arising from hair follicle showing predilection to sun exposed hair bearing skin.Eyelid is commonly affected however it is a rare site compared to scalp and face.We report a case of biopsy proven trichilemmal carcinoma which was initially suspected to be basal cell carcinoma.A 72 year old male presented with complaints of growth in the lower lid of left eye which was present for the past 3 years.On examination,a firm pigmented mass fixed to the underlying structures was present.A provisional diagnosis of basal cell carcinoma was made.Incisional biopsy done.The biopsy report revealed pigmented desmoplastic trichilemmoma and warranted complete excision to rule out malignancy.Wide excision of the mass with lid reconstruction was done and the biopsy report revealed invasive trichilemmal carcinoma of clear cell type.Margin clearance was ensured.This tumour has a low potential for metastasis.The patient was found to be doing well on his follow up visits.
