55 yr old female presented with a gradually enlarging painless mass in left upper lid for 6m with no history of trauma, any other swelling or systemic illnesses. BCVA was 6/6 OU. Clinical examination revealed 15×7 mm solitary, irregular, firm, non-tender mass on medial side of left upper lid with overlying atrophic skin and mechanical ptosis. Anterior segment and dilated fundus examination were normal OU. Systemic investigations and peripheral smear were normal. CECT showed homogenously enhancing soft tissue mass in left superomedial orbit causing LUL ptosis, suggesting lymphoma. FNAB revealed polymorphous lymphocytes. In absence of a definitive diagnosis, steroids were avoided. Finally excision biopsy ruled out lymphoma. Orbital pseudolymphoma is a rare benign tumor, typically presenting as painless swelling , ptosis, proptosis or diplopia. Misdiagnosis is common and hence necessitates biopsy. Treatment options include excision, corticosteroid therapy and EBRT.
