A 58 year old male patient came to the out patients department with complains of both eye visual loss and left eye proptosis for the past 2 months. B/E Best Corrected visual acuity was PL+ and PR inaccurate. On examination B/E showed Senile immature cataract. Fundus Examination revealed disc pallor with arteriolar attenuation and peri-papillary atrophy. B-scan was done which showed irregular hyper-echogenicity in the retrobulbar region. CT scan was done, which showed expansion and sclerosis of skull bones including skull base with associated soft tissue component causing forward protrusion of left globe. Orbital walls are involved circumferentially resulting in narrowing of optic foramen leading to compression of the optic nerve, suggestive of Paget’s disease with sarcomatous changes.
