Aim:To explore the outcome of using optical grade tissues in recalcitrant corneal ulcers.
Methods: It is a prospective study on therapeutic keratoplasties in 22 eyes using optical grade tissues. Donor tissues with endothelial cell count of more than 2000cells /mm were used.
Results:The mean best corrected pre-operative and post-operative visual acuity were 1.54 logMAR and 0.90 log MAR respectively (P=0.0002). The mean ulcer size was 5.42mm2.Out of22, 14 (63.6%) cases were positive for fungus, two(9%)for bacteria and five (22.7%) cases showed no organisms.Reinfection occured in three(13.6%) patients,one underwent a second therapeutic keratoplasty and another case was treated with intrastromal, intracameral antifungals and broad iridectomy.Graft failure occurred in one(4.5%) patient.
Conclusion:Performing therapeutic keratoplasty with optical grade tissues for corneal ulcers provides both,therapeutic and optical treatment,avoiding a second surgery reducing overall patient’s morbidity.
