AIM-To evaluate outcomes of management of encapsulated blebs.
METHODS-Retrospective analysis of patients with encysted blebs following glaucoma filtering surgery(GFS)(2009-2016).Details of previous surgeries, IOP lowering medications, modes of management and outcomes analysed.
RESULTS-57eyes(57 patients)-37 post trabeculectomy and 20 post phacotrabeculectomy were included.Mean age was 48(SD 3.5)years.Median duration between GFS and noting bleb encystment was 1.53 months(IQR 1.17-2.97).Mean follow up was 18.9±17.7months. 9 eyes were treated with globe massage,27 eyes with aqueous suppressants and 21 eyes with bleb needling as primary treatment, with long term success rate of 77.8, 48.2 and 76.2% respectively. Encysted blebs with failure after primary management (OR:3.7,CI:1.1-13,p=0.036)were prone to fail in long run.
CONCLUSION-Bleb needling is preferable over aqueous suppressants in encysted bleb management. Immediate outcome following primary management can predict long term outcome.
