A 52 yrs female presented with complaints of recurrent swelling in the lower lid LE for 5 months. The swelling was insidious onset, gradually progressive, associated with dull pain. It was firm mass measuring 1.5x1cm involving the lid margin and the tarsal conjunctiva with irregular surface. No regional node involvement. She gave history of I&D done twice elsewhere. With suspsicion of a Ca, Biopsy from the lesion was done which was suggestive of sebaceous gland cell carcinoma. The patient underwent excision and biopsy with a clearance of 3 mm margin. The lower lid was reconstructed with canthotomy and cantholysis. The mass and the tissues from margins of excision were sent for HPE which confirmed our diagnosis and the margins were free of tumour cells. The patient is in follow up post operatively with no complications. We present this case to emphasise the role of HPE in recurrent chalazion and surgical intervention with tumor free margins in cases of sebaceous gland cell carcinoma.
