Iridocorneal endothelial syndrome is primarily corneal endothelial abnormality syndrome. It is typically unilateral, commonly affecting young and middle- aged woman. Three pathological features which are involved in this syndrome are an abnormal corneal endothelial cell layer ,trabecular obstruction by proliferating tissue causing glaucoma, and iris abnormalities. Here, we report a case of Iridocorneal endothelial syndrome (Essential Iris Atrophy) in 35 years old female who presented with defective vision since 4 months in left eye.Left eye vision was 6/12 and intraocular pressure was found to be 44mm of mercury by goldman’s applanation tonometer. Corectopia, pseudopolycoria and atrophic patches were present in iris. Right eye was clinically normal. Patient underwent trabeculectomy in left eye following which left eye intraocular pressure was brought under control that is 18mm of mercury.
