Method:A 50 year old female presented with complaints of sudden onset headache, swelling and bulging of right eye since 8 days. O/E vision was 6/24-618, lid edema, conjunctival chemosis, conjunctival congestion, circumcillary congestion and proptosis was present in right eye.Ocular movements in all the gaze were restricted. MRI brain with orbit with contrast MR angiography was suggestive of right carotid cavernous fistula with right eye proptosis with edematous and enlarged extra-ocular muscles, retrobulbar edema and prominent superior ophthalmic vein.
Result:Embolisation of carotid-cavernous fistula was planned and done.On 3rd follow up day, there was reduction in symptoms. Patient as discharged on oral corticosteroids, antibiotic drops & lubricating drops. On later follow ups there was complete resolution of symptoms.
Conclusion:The goals of treatment are to eliminate the fistula & preserve carotid artery patency,because cortical venous drainage and a risk of haemorrhage are present
