PURPOSE:To compare the visual outcomes and complications of temporal hinge LASIK with superior hinge LASIK.DESIGN: Retrospective case-control.METHODS:Two groups-1)temporal or 2)superior hinge using IntraLase Femtolaser or ML7 microkeratome. Wavefront guided ablations using VISX StarS4. Visual acuity, contrast sensitivity and complications analysed at 1 week,1mnth and 3mnths.RESULTS:Significant difference in mean VA between the temp and sup groups at p< 0.5 and in contrast sensitivity at p< 0.01.In the sup hinge group, there was a greater incidence of limbal bleed as compared to temp.CONCLUSION:Temp hinge is comparable to superior. It offers advantages as a temp hinge enables the surgeon to make a larger flap diameter avoiding intraop bleeding and reduces the chances of inadvertent flap ablation at the hinge. It becomes possible to increase hinge width for better mechanical stability. It reduces the intraop interaction between the flap and the upper lid margin and lashes, reducing infections
