Congenital cataract requires early treatment to prevent permanent loss / decreased vision (amblyopia). In this case report, there are 2 aspects – limited resources and the neglected female child in India. A 15 year old girl presented with a white reflex and decreased vision in the left eye since birth. The R/E had a vision of 6/9, with a congenital cataract. The vision in the left eye was limited to Fc at 1 meter, with exotropia of 30 ∆. A dense congenital cataract was seen on Slit-lamp examination. Fundus in both eyes was not visible. Ultrasound B-scan and intraocular pressure was normal both eyes. Right Eye cataract extraction with an implantation of a foldable hydrophilic lens(due to affordability) in-the-bag was done. Post operative topical antibiotic and steroids and oral steroids were given. Post op vision in right eye was 6/18 and exotropia reduced to 15∆ . There should be an emphasis on early treatment of congenital cataract and equality of the girl child for best results.
