69 year old male presented with swelling on the lateral side of right eye and binocular diplopia intermittently since four months. The lesion was excised with surrounding 5mm conjunctiva under topical 0.5% proparacain hydrochloride drops. The base of the lesion was attached to the lateral rectus muscle sheath. 5% povidone iodine was applied to the excised area. Histopathological examination of the lesion showed findings consistent with the rhinosporidiosis. No recurrence was noted at last follow up 12 months after excision of the lesion. This case highlights the attachment of lesion to the lateral rectus and possible use of povidone iodine in conjunctival rhinosporidiosis, which should be further investigated.
