Dr.Prangya Panda
A nine year old irritable female child presented with watering and foreignbody sensation both eyes. Both eyelids of both eyes were turned in. The lid margins along with eyelashes were rubbing over cornea. There were no lid crease. Cornea was opacified and keratinised.
Till the patient was fit for GA leucoplast was attached everting the lid margin to the orbital rim. Antibiotic e/d artificial tear drop instilled. Surgical correction was done under GA. Lower lid- minimum ellipse of skin and orbicularis was excised. Skin was fixed to the lower edge of tarsus and wound closed. Upper lid- The anterior lamella of skin and muscle is undermined, repositioned and sutured to the tarsus at a higher level. It is also sutured to the aponeurosis with the skin closure sutures. This reforms the skin crease.
Discussion and conclusion- here there is disinsertion of lowerlid retractors and secondary overaction of marginal orbicularis. There was weakness of tarsal plate.
