Epiphora is a common condition in ophthalmological practice,a result of decreased tear drainage.Inadequate tear drainage is either caused by a pump failure or an anatomical obstruction along the lacrimal drainage pathway. There is subgroup of patients with epiphora exhibiting decreased tear drainage and increased tear line associated with normal pump function and a patent lacrimal system This status can be named as FUNCTIONAL NASOLACRIMAL DRAINAGE OBSTRUCTION .Most cases in our set up show anatomical obstruction .FNLDO cases are difficult to diagnose . A 42 year old male presented to us with history of swelling over right lacrimal sac for last 2 years along with epiphora. CT , MRI ,MRA (angiogram) were done . Radiographic pictures raised suspicion of Right Sided Lacrimal Mucocele / Dacryocystocele/Malignant Mass of 25 ×25 mm .But we found NLD patency to be fully maintained .Patient has been planned for Surgical Exploration / Ext-Dacryocystorhinostomy.
