Purpose:To highlight the etiology, manifestations and outcome of algorithm-based management of severe contracted socket
Method:Retrospective review of 32 consecutive patients with contracted socket from January 2013 to December 2016
Results:Mean age was 30.6 ± 17.6 y. Twenty-six patients(84%) had anophthalmic contracted socket, post-enucleation(17, 65%) or evisceration(9, 35%); 3 (10%) had congenital microphthalmos; 2(6%) had phthisis bulbi. 4 patients(13%) had received radiotherapy. All had volume loss, while 27(87%) had surface loss. Socket was dry in 3(10%). Primary management included dermis fat graft (DFG) with fornix formation sutures (FFS) in 17(55%), mucous membrane graft (MMG) with FFS in 5(16%); socket expansion with serial conformers in 4(13%). At a mean follow-up of 9.9 ± 9.66 m, 27(87%) had good anatomical outcome with optimal prosthetic fitting.
Conclusion:Systematic and staged management helps achieve good anatomical and functional outcomes in severe contracted socket.
