Purpose: To report a case of good visual recovery after complete loss of Lasik flap, 1 year following Lasik. Methods: A 24 year old male patient presented with a history of tangential injury to the left eye with a tree branch. He had undergone uneventful microkeratome assisted laser in situ keratomileusis in both eyes 1 year before the injury. On evaluation he was found to have complete flap avulsion with debris in the left eye, with uncorrected visual acuity (UCVA) of 3/60. The eye was thoroughly irrigated and he was treated with topical antibiotics and lubricating eye drops. Results: The epithelium healed completely over 1 week with mild haze. He was started on topical fluorometholone 0.1% for 4 times a day. At 1 month of follow up, the uncorrected visual acuity was 6/6p. Conclusion: Traumatic flap loss may cause irregular astigmatism and unpredictable refractive change. This case illustrates excellent recovery from a potentially sight threatening event following refractive surgery.
