Purpose: To evaluate biomechanical and confocal microscopy (CM) changes in myopia and correlate with corneal thickness
Methods: Prospective evaluation of 204 eyes (102 patients) with myopia ≥ 0.5 D planned for laser refractive surgery. Eyes were divided on basis of thinnest pachymetry (CT) -group I: CT ≤500 μm, group II: CT >500 μm. Corneal hysteresis (CH), corneal resistance factor (CRF) and CM findings were evaluated.
Results: Mean age was 22.78 ± 2.43 years. 57 eyes had CT ≤500 μm and 147 eyes had CT >500 μm. Group I had significantly lesser mid- stromal keratocyte density (SKD) (p=0.01), CH (p=0.01) and CRF (p= 0.004) than group II. Corneal biomechanics (CH and CRF) significantly correlated with CT (r=0.45 & 0.36, p<0.001) and anterior SKD (r=0.4, p<0.001). There was poor correlation between magnitude of refractive error and CT. Conclusion: Thinner corneas (≤500 μm) have suboptimal corneal biomechanics and keratocyte density as compared to thicker corneas.
