Purpose: To evaluate medial transposition of split lateral rectus(LR) in cases of complete third nerve palsy
Methods: Consecutive patients of third nerve palsy presenting to our centre over a period of 1 year treated with medial transposition of split LR were included in the study. Post- operative deviations were noted at 3 month interval.
Results: 7 patients were included, out of which 4(57.1%) were male. The mean patient age was 28±18.46 years.The pre-operative horizontal deviation was >90 prism dioptres (PD) in 6(86.7%) cases. At the end of 3 month follow-up, the mean post-operative deviation was 15.71 ± 8.86 PD . 2 patients had a post operative under correction > 20 PD. 1 patient had pre-operative synergistic divergence which was corrected by this procedure.
Conclusions: LR to MR transposition can be used as a safe procedure to achieve cosmetically acceptable results in cases of complete third nerve palsy and is well suited for cases of synergistic divergence.
