Purpose: To present a case of bilateral severe strabismus fixus convergence in a high myopic patient surgically treated by loop myopexy of superior rectus (SR) and lateral rectus (LR) along with medial rectus recession.
Methods: A 65 year old high myopic female patient with an axial length of 33mm presented with inward and downward deviation of both eyes with severe limitation of extra ocular movements in all gazes. On computed tomography of the orbit, there was nasal displacement of SR and inferior displacement of LR with protrusion of globe in the superotemporal quadrant in both the eyes. Patient underwent both eye loop myopexy of SR and LR with scleral fixation along with medial rectus recession.
Results: Post operatively there was resolution of esotropia in primary gaze with improvement of extra ocular movements in all gazes.
Conclusion: Myopic strabismus fixus is a rare abnormality of ocular motility. Loop myopexy is now the most preferred surgery.
