indications for surgery:1) vertical deviation in primary gaze 2) deviation induced amblyopia 3) diplopia 4) restricted binocular field.
goals of surgery: 1) improve the position of affected eye in primary gaze 2) increase binocular field
material and methods: choice of surgery was based on FDT results intraop. 11 out of 16 patients had positive FDT with IR restriction, underwent IRc. 5 had free FDT with elevation weakness underwent Knapp”s. IRc was followed by Knapp”s for persistent hypotropia after IRc with SR palsy.
results: out of 16 patients, 13(88.9%) were corrected within 10pd of orthophoria. 1 had overcorrection and 2 had undercorrection. there was a mean correction of 19.7PD in the amount of hypotropia in primary gaze from the preop value of 25.8PD. the mean postop vertical deviation was 6.11+/-7.9 PD
conclusion: surgery for MED should be individualized based on FDT.
