BACKGROUND:Orbital cellulitis per se by either Bacillus cereus / Rhizobium has not been reported in literature so far.
OBJECTIVE: To report orbital cellulitis in a 5 year old by Bacillus cereus secondary to periocular injury by soil contaminated firecracker.
CASE REPORT:A five-year-old boy presented to us as right preseptal cellulitis with foreign bodies in the superomedial aspect following firecracker injury. Following foreign body removal the child precipitously developed gross proptosis and optic neuropathy due to orbital cellulitis. Microbiological examination revealed Bacillus cereus and Rhizobium radiobacter. His signs improved following treatment with intravenous clindamycin and piperacillin though his vision failed to improve beyond light perception.
CONCLUSION: Trivial periocular injuries with virulent pathogens can lead to significant morbidity.Prompt therapy must be initiated early for the speed with which irreversible changes occur makes it difficult to salvage vision.
