AIM: Study of complications and visual outcome following globe perforation due to periocular blocks or bridle suture.
METHOD: Retrospective analysis of cases between January 2008 and December 2016
RESULTS: Incidence was similar: 22 (0.0086%, 254157 cases) following periocular blocks (retrobulbar 4, peribulbar 18) and 17 (0.009%, 187713 cases) following bridle suture. All 39 cases had a soft eye, and 32 had vitreous haemorrhage. Breaks were predominantly inferotemporal (blocks), and superior (bridle suture).
Barrage laser was done in 9 cases, cryopexy in 3, and vitreo-retinal surgery in 6 cases, while 14 cases were treated conservatively. But 7 cases did not undergo advised treatment
Fifteen (38%) patients achieved a vision of 6/18 and better, 13 (33%) had vision less than 6/60 and 16 patients (41%) had no improvement or worse vision than at presentation.
CONCLUSION: Safer techniques like Sub-tenon’s anaesthesia, and avoidance of bridle suture may help reduce unnecessary loss of vision
