A 30 year old male patient of road side accident was examined 3 days post trauma for sudden unilateral loss of vision. Complete anterior and posterior segment examination including slit lamp examination, indirect ophthalmoscopy, slit lamp biomicroscopy with 78D lens, fundus fluorescein angiography was done. Subconjunctival haemorrhage was found in both eyes and rest of the anterior segment examination was normal. Multiple white patches in the peripapillary area and around the macula and few flame shaped hemorrhages was found in left eye on fundus examination. The patient responded well to intravenous methylprednisolone followed by oral steroids in a tapered manner.unilateral presentation of purtscher’s retinopathy was seen. Patient was given systemic steroids and followed up. It responds well to oral steroids and vision improvement is significant. Purtscher`s retinopathy as a unilateral presentation is not commonly seen hence we are presenting this case.
