A 66 year old male from rural area attended EYE OPD with c/o blurrring of vision for long time which increases for last 3 days.Along with deep pain in both eyes. BCVA at presentation was 5/60 OD, 2/60 OS.b/l NVI and NVA seen in slit lamp examination and gonioscopy. IOP 30OD,28OS.Fundus examination revealed B/L CRVO.CMT in OCT 383 mu OD and 959 mu OS.On general physical examination patient appeared normal with BP 160/100,PR 88/min regular. He was not on any hypertensive medications before.Patient advised for all routine investigation along with coagulation profile, protein C,protein S, VWF analysis. All reports came in normal limit. Bone marrow biopsy also turned out to be normal. So our diagnosis is – it is a case of B/L CRVO due to uncontrolled hypertension.Patient started anti hypertensive medications. B/L IV Ranibizumab given 2 injections each eye.But vision did not improve much.so we concluded that Uncontrolled hypertension can present as a severe B/L CRVO like situation as well.
