Purpose: To report three cases with established glaucoma who presented with an acute neurological event, which was diagnosed by visual field testing. Materials and methods: The 3 patients, who were being treated for established glaucoma, presented with apparent worsening of visual complaints. Field analysis using Humphrey field analyser was done. Results: New field changes were found to be overlapping the pre-existing glaucomatous defects. MRI showed an acute vascular event in the middle cerebral artery territory in all 3 cases. Conclusion: Occurrence of a visual field defect may sometimes signal an acute cerebrovascular accident. Hence, deterioration of visual fields in patients with glaucoma, may be due to neurological events, as seen in our patients, and not merely due to worsening or progression of the glaucoma.
