Until recently, penetrating keratoplasty in early neonatal period was viewed in disfavour due to the technical, surgical and post-operative difficulties. The common indications include congenital corneal opacities, traumatic perforations and keratitis.
Here we present a venturous case of TKP done on a 2 day old preterm (34 weeks) neonate delivered by emergency LSCS due to PPROM referred with C/o of RE redness and discharge. O/E, RE had an iatrogenic corneal tear temporally with infiltration on either side with shallow AC. With the opportune availability of donor cornea from a 3 day old child, we proceeded with RE TKP on the same day in ICH using ENT microscope, under expert neonatal anaesthetic care. Corneal button C/S yielded growth of coagulase negative Staphylococcus. Blood culture revealed Acinetobacter baumannii. This emergency TKP done for the 2 day old neonate with a 3 day old child’s cornea makes it exquisite and is one of the few early neonatal TKPs done in South India.
