In our routine clinical practice at our eye bank in our institute we occasionally come across a situation where we have a good quality corneal tissue available for keratoplasty however the mate corneal tissue of the same donor has a corneal infiltrate.
As a result we analyzed the data of one year at our eye bank where a clear donor cornea was used for keratoplasty in a setting of mate corneal tissue having corneal infiltrate.
Out of a total of 2188 keratoplasties, 22 were performed with a clear donor cornea where the mate corneal tissue had an infiltrate. No corneal infiltrate or adverse event was reported in 21/22 cases, however one patient developed endophthalmitis one day day following endothelial keratoplasty and Klebsiella was isolated from both the rim of the used cornea and unused mate cornea on microbiology.
We concluded that there is a 4.5% risk of having an adverse event in using a corneal tissue for keratoplasty in a setting of mate cornea having an infiltrate.
