A 55/M, with bilateral chronic central serous chorioretinopathy (CCSCR) was treated with 3 intraviteal (IV) ranibizumab and 1 IV aflibercept in OD over 5 months. His BCVA remained unchanged in OD (6/12) & OS (6/60). In view of the recalcitrant nature of the disease, he was switched to IV Ziv-aflibercept in OD. After 1 month, he showed significant improvement in BCVA, both in OD (6/9) and OS (6/36). Spectral domain optical coherence tomography (SD-OCT) of both eyes illustrated notable reduction in intraretinal fluid, subretinal fluid and central macular thickness. Enhanced depth imaging OCT also demonstrated bilateral significant reduction in choroidal thickness and choroidal area. Our case report is unique as it exclusive demonstrates the efficacy of IV ziv-aflibercept in CCSCR. Additionally, for the first time in literature, we illustrate that IV ziv-aflibercept has bilateral effect after unilateral injection, possibly due to systemic absorption, which needs to be further investigated
