A 49 year old female came with painless, gradually progressive diminution of vision in left eye since 5 years. On ocular examination the best corrected vision of right eye was 6/6 while the vision of left eye was 5/60 with no improvement. The anterior segment of both eyes showed no abnormality. Dilated fundus examination of right eye was within normal limits while that of left eye showed a well circumscribed optic nerve head pit occupying about 1/6th of optic disc area with rim loss at the inferotemporal margin associated with a wide area of RPE disturbances along with presence of thin epiretinal membrane over the macula in the inferior half around 5 disc diameter suggestive of resolved long standing CSCR. This findings was confirmed on OCT which showed the evidence of optic disc pit and foveal thinning with CMT of 21microns. Hence in patients with CSCR, the presence of associated optic nerve head pit should be looked up, to plan different treatment strategies.
