Aim: Evaluate safety of uveo-sclerectomy via postero-anterior approach (cyclo-iridectomy) for peripheral iris melanoma
Method: Two patients presented with mass protruding from iris. Contrast-enhanced MRI was suggestive of iris melanoma
Superior conjunctival peritomy was performed. A lamellar sclera flap, hinging anteriorly was made such that it extended into cornea, so that normal iris anterior to tumour was visible. A deep sclerotomy was made within 1-2mm of outer scleral incision. An iridotomy was made adjacent to iris root and extended around anterior margin of tumour and back to angle on other side of lesion
Result: Both patients maintained vision with no recurrence of tumour. Transient hypotony was noted but resolved within a week. Histopathology revealed iris melanoma in one patient and epithelial inclusion cyst in other
Conclusion: Surgical resection of peripheral iris melanoma achieves visual conservation with local tumour control, providing sample for histopathology.
