Visual axis opacification and IOL decentration are the two most common complications associated with pediatric traumatic cataracts. We demonstrate the technique of posterior optic capture of foldable PCIOL in variety of penetrating pediatric traumatic cataract cases after primary posterior capsulotomy. The advantage is to eliminate the risk of visual axis opacification, decrease co-morbidities and to maintain well centered IOL. Preserving all the benefits of microincision cataract surgery. The long term outcome is demonstrated
