This video demonstrates thesurgical technique of macular holesurgery without gas tamponade orpostoperative posturing inlarge macular holesThe surgical technique included pars plana 23 gauge vitrectomy with broadinternal limiting membrane peel (ILMpeel), inverted ILM flap repositioning(ILMR) and use of autologousgluconated blood clumps (AGBC) asa macular plug to close the macularhole. No fluid air exchange,endotamponade or postoperativeposturing was used.We also demonstrate the efficacy of utilising a dislodged ILM flap as an autologous graft. The serial SD – OCT changes demonstrating the restoration of the outer retinal layer integrity in the postoperative period is also provided.
