A 38-year-old male with a metallic intraocular foreign body (IOFB) embedded in the ciliary body presented to us with BCVA of 6/6 and early ERG changes of metallosis. The IOFB mandated extraction to prevent long-term sequelae. The challenges included visualization of IOFB and approaching it without damaging the crystalline lens. A novel technique of indentation with one port open during 25 gauge PPV was employed. Further, the IOFB was delivered using a double handshake technique, thus protecting the lens. At 6 weeks postoperatively, BCVA remained 6/6 with a clear lens.
