Pars plana 23G vitrectimy is a very good option to manage vitreous prolapse in the anterior chamber in case of posterior capsular rent .Nuclear fragments and IOL can be made free of vitreous entanglements by ppv in the anterior vitreous .Thereafter it can be levitated into the anterior chamber. The sunken nucleus/IOL could be seen loitering freely in mid vitreous.W hen a gush of aqueous is let off from the sclero corneal tunnel by depressing the posterior lip of the wound the nucleus/IOL comes up on its own along with the aqueous flow. The situation was salvaged…..somewhat dramatically.
Manual lamellar sclera-corneal tunnel is an excellent means to deliver the nuclear fragments out of the eye.IOl is placed in the sulcus and captured beyond the capsulorehxis.
