Boston keratoprosthesis with modified titanium backplate implantation is required for vision rehabilitation in high risk corneal recipient beds. This instructional video deals with the surgical difficulties encountered in the Boston keratoprosthesis with modified titanium backplate. Special concerns in preoperative evaluation, investigations, intraoperative surgical steps, postoperative concerns in a series of 10 patients will be depicted. Particular concerns in postoperative complications and approaches to their management will also be highlighted. Surgical approaches to optimal implantation of the keratoprosthesis, its assembly and placement will be shown in detail. Practical tips to deal with the postoperative concerns relating to posterior segment problems will also be covered. The long term postoperative outcome in this clinical series will depict the success rates, complications, special problems and managmenent concerns in this special corneal surgical scenario.
