Synopsis: Cysticercosis is a common infestation in our country. Intraocular cysticerosis has been sparsely reported and submacular cysticercus has been rarely described. However surgical treatment of a submacular cysticercus has not been reported. We came across a case with 3-disc diameter sized live encysted cysticercus underneath the fovea with BCVA of 2/60. Waiting for spontaneous breakthrough of the cysticercus through fovea was fraught with poor visual outcome. We performed 25 g PPV with macular retinotomy and cysticercus removal. Postoperatively the patient had a BCVA of 6/24. We present this video with an aim to discuss this novel technique as well as promote early intervention in subfoveal cysticecus.
